Personal Transition Plan
A requirement for all Hawaii public high school graduates, the Personal Transition Plan (PTP) is each student's plan of action to transition from high school to college and careers.
The elements of the students' personal transition plan are:
- goal attainment
- identification of available resources
- evidence to support the plan of action taken
- self-evaluation component
Student, parent, and school personnel have a shared responsibility in the development and execution of the PTP during high school.
The table below shows the progression and completed tasks/components expected at each grade level. The PTP (0.5) graduation credit represents FOUR YEARS of college and career preparation. At Kauai High School, students in grade 9 are scheduled in a Career and Academic Planning class and students in grades 10-12 are scheduled in a homeroom class which is used to provide students with time during the school year to complete specific grade level PTP tasks. A student’s homeroom grade of an S or U will indicate progress made for expected grade level portions. The final PTP, graduation required credit, will be awarded upon the students first semester of their 12th grade year or the ending of their 11th grade year provided all required PTP components from all grade levels are completed.

*Completion of the Personal Transition Plan (PTP) is required to all school year graduating class members in order to have a modified schedule as well as to be able to be a Teacher’s aide for one of their periods in their schedule.
